Incident Investigation still constitutes one of the most powerful tools for organisational learning.

Many industries have seen improved business and safety performance in the preceding decades. However, even though incident rates have been maintained or lowered, the opportunity to challenge prevailing assumptions remains anchored in the quality of learning through incident investigations.

Harvesting the learning potential from such occurrences beyond any immediate findings still remains challenging. Many reports remain stuck in the superficial details of the last incident and judgments of human performance rather than explanations for why actions and assessments made sense at the time.

This unique three-day course is designed to address exactly this. Combining a total of more than three decades of industry experience with the most prominent thinking in contemporary safety science enables participants to move beyond human error and identify the deeper levels of organisational behaviour that shape individual performance for the benefit of the organisation and industry.

Rather than focusing on methodology, the course applies theory, case studies and exercises to take participants through a reflective discovery process, providing a different understanding of incidents.
